Joining FORMS

** All forms must be printed on 11x14 inch Legal paper.

All new cadets please complete the following forms to join 330 Air Cadets.

2. CF1158 Cadet Membership Form

In addition to this form, Cadets will need to provide a copy of citizenship (birth cert, pr card, passport) and health plan coverage (OHIP or private plan card). Please use the provided checklist (pg.1) to ensure you have filled out all areas.

Cadets with non-allergy based medical issues will be required to fill-out a DHQ. This to be provided by Admin Officer after enrollment.

2. Detailed Health Questionnaire (DND 2570 09-2012)

If your child has a medical allergy, please complete Forms 3-6 below.
